I will respect the confidentiality and privacy of all the information shared/obtained from Jeet Nursing Prep Center (JNPC).

VERIFICATION OF OBLIGATION

I understand that in the course of my experience with JNPC, I may have access to and be involved in the processing of verbal, written, computer generated, computer accessed, filmed, and/ or recorded information related to study material, clients, employees, and staff or the organization or business.

I understand that I am required to maintain confidentiality of this Course and course related information provided by JNPC at all the times, both during and after my tution period.

I understand that I will not share, discuss, or reveal any of this information with anyone.

I understand that any breach of confidentiality may result in disciplinary action, including termination from the current/ future programs (without getting any refunds or other benefits) or legal action.

I agree to compensate JNPC / Sole Proprietor for losses it incurs as a result of my negligent, reckless, or intentional disclosure of Confidential Information, meaning that I will pay for any damages I cause, including legal costs.

I give consent by my signature that acknowledge being informed of the confidentiality policy concerning confidential information or its treatment.

Information, resources and branding at Jeet Nursing Prep Center is covered under the Copyright. Act (R.S.C., 1985, c. C 42) and the Trademarks Act (R.S.C., 1985, c. T -13 s. 1; 2014, c. 20, s.318 (E)

JNPC SOCIAL MEDIA CONSENT FORM

Jeet Nursing Prep Center (JNPC) collects, uses, discloses and retains personal information in compliance with the Canada Freedom of Information and Protection of Privacy Act (the FIPPA).

JNPC takes photographs of, and interviews individuals for the purposes of publicity and business promotion in various media (e.g., printed materials, website, social media). The collection of this personal information is permitted under Section 26 (c) of the FIPPA.

I further consent to the reproduction or use of the photographs /information with or without my name, and consent that JNPC may seek copyright of the photographs / information in their name.

In giving this consent, I release JNPC and its nominees from liability for any violation of any personal or proprietary right have in connection with any sale, reproduction or use of the photographs/ interviews.

I certify that I am 19 years of age or older and consent to being tagged on social media.
